Our nurses, under the supervision of your physician, will ensure to check if the medicines are working and controlling symptoms appropriately. They will monitor your medication and ask for immediate help if there’s a change in your health status.
Medication management services may include:
- Assist in creating a medication schedule
- Reviewing the medication regimen
- Creating a complete list of all prescribed and over-the-counter medicines the patient is taking
- Educating patient and/or caregiver on proper dosage, frequency, and side effects
- Evaluating response to medications and effectiveness
